Here's what you need to know

Aflac's Group Supplemental Dental Insurance provides benefits for periodic checkups and cleanings, X-rays, fillings, crowns, and much more. As a new hire, you can enroll within 30 days of your benefits eligible date.

 

It's your smile and your plan; Aflac's Group Supplemental Dental Insurance gives you control.

 

  • You choose your dentist. Because Aflac doesn't use a network of dentists, you can go to any dentist you choose.
  • You and your dentist choose the best treatment for you. Aflac's GroupSupplemental Dental Insurance doesn't have precertification requirements. If the treatment is covered by your plan, you don't need Aflac's permission to receive it.

Aflac's Group Supplemental Dental Insurance is different from many other dental plans you may have seen.

 

  • You know what you're getting with Aflac's Group Supplemental Dental Insurance. The plan spells out the benefits for both wellness and other diagnostic/treatment services. There are no gray areas. Each covered procedure has a specific benefit amount.
  • Aflac's Group Supplemental Dental Insurance doesn't have an annual deductible. Other dental plans may require you to meet an annual deductible before benefits are payable.
  • Aflac's Group Supplemental Dental Insurance pays benefits regardless of any other plan. Even if you have other coverage, you'll receive your full Aflac benefit amount.

These are the periods when you are eligible to enroll for Aflac's Group Supplemental Dental Insurance:

 

  1. Within your new hire enrollment period
  2. If you have a qualifying life event (you get married, have a baby, etc.)
  3. During your employer’s annual enrollment

 

Check with your employer’s human resources/benefits team for specifics about these timeframes.

 

Keep in mind: If you enroll during these timeframes, you have what is called "guaranteed issue," which means you may qualify for coverage without answering health questions. You must have coverage in place before a covered dental exam or procedure happens in order to be eligible to collect the benefit when you need it. Please note some benefits have a waiting period before benefits may be payable.

    Members of your family who are considered eligible to enroll for coverage include:

    • You – if you are an active employee working 16+ hours per week
    • Your legal spouse - age 18 and older
    • Your child(ren) – under age 26 

     

    You must be enrolled in coverage for eligible members of your family to also enroll.

    No, you don’t! If you enroll during your eligible enrollment periods, you have "guaranteed issue," which means you may qualify for coverage without answering health questions.

  • What if my employee status changes?

    Good news! If you leave or retire from your current employer, you can continue your coverage without interruption (subject to applicable law and the plan’s terms and conditions). Although payroll deduction will no longer be available, you can opt for other payment methods such as direct bank account deduction, credit card billing or home billing. Higher rates may apply.
